    Menopause is the permanent end of menstrual cycles, and it typically occurs between the ages of 45 and 55. As hormone levels shift, many people experience hot flashes and mood changes. Hormone replacement therapy (HRT) addresses these changes by supplementing estrogen, progestogen, or both. Here are some ways HRT is used to manage menopause symptoms: 

    Systemic Hormone Replacement Therapy

    Systemic HRT is designed to deliver hormones throughout the entire body. Since the hormones travel through the bloodstream, they reach tissues across the body rather than staying confined to one area. This makes systemic HRT a common choice for widespread symptoms; they include hot flashes, night sweats, and mood fluctuations. 

    Estrogen forms the foundation of many systemic HRT regimens. For people who still have a uterus, a clinician typically adds progestogen to protect the uterine lining from the effects of estrogen alone. Dosages are not one-size-fits-all; a clinician tailors them based on your symptoms, medical history, and how your body responds over time. Adjustments are made gradually, and regular check-ins help track progress and flag concerns early.

    Some people start systemic HRT at the onset of menopause, while others begin treatment after symptoms become more disruptive. Because treatment must account for individual health factors, its timing and format are not the same for everyone. A clinician will walk you through the available options, explain the associated risks, and help you weigh them against your circumstances. Honest, open communication with your healthcare provider makes the process more manageable.

    Low-Dose Vaginal Therapy

    Low-dose vaginal therapy is a localized option that directly targets specific symptoms. A small amount of estrogen is released into the surrounding vaginal tissue, and very little of it enters the bloodstream. It is primarily used for menopause symptoms like vaginal dryness, irritation, and discomfort during intercourse. These are symptoms that systemic therapy does not always fully address. Available formats include:

    • Creams: These follow a frequent application schedule, often starting with daily use before tapering down.
    • Tablets: Small dissolvable tablets are inserted into the vagina using an applicator. Many people find them easy to use and less messy than creams.
    • Vaginal rings: Soft rings placed inside the vagina stay in place for several weeks at a time; this offers a low-maintenance option. 

    Because the hormone dose is low and remains localized, this method is sometimes used alongside systemic HRT or on its own when systemic treatment is not appropriate. Your clinician will help determine which format suits your lifestyle and symptom pattern. Your symptoms may change as treatment continues, and ongoing evaluation helps keep your care on track.

    Progestogen-Alone Therapy

    Progestogen-alone therapy does not include estrogen, so it is an option for people who cannot take estrogen or prefer to avoid it. Tablets, injections, and intrauterine systems are available delivery methods, each following a different schedule and administration approach. In combined HRT, progestogen serves to balance the effects of estrogen and protect the uterine lining. When used alone, it primarily helps manage irregular or heavy menstrual bleeding during perimenopause. Not everyone responds to progestogen-alone therapy in the same way, so a clinician reviews your full medical history before recommending this path. Monitoring continues throughout treatment to confirm the approach remains appropriate as your needs evolve. 

    Manage Menopause Symptoms

    Hormone replacement therapy encompasses a range of approaches, from systemic treatments to localized applications, each working in distinct ways. The right path depends on your symptoms, medical history, and personal preferences. A clinician reviews all these factors and outlines the benefits and risks tied to each option. Treatments are not static; they are reviewed and adjusted as your circumstances change.
